Butchering canon. Actually, I'm a bit torn on this - I have zero tolerance for changing character personalities, but "what-if" scenarios (e.g. what if the bad guy won at plot point X, or a character was born in a different place) can be good as long as they're plausible in that universe and aren't specifically meant to cater to an OC. Basing a story on canon before a certain point and ignoring what happened afterward (e.g. my "I Am the Eggman" story) can be interesting, but - again - I have no respect for people who do that to help a character and not a story. (And everyone who does both - usually by creating an OC/canon pairing that would never work for reasons established in canon - needs to be shot in the writing arm. Yes, I am referring to everyone who's ever paired Sasuke, Loki, Wesker, or Voldemort with anyone.)
Lack of overall research. I find it so insulting when you have a character with a mental problem but just butcher the hell out of it and make it as insensitive as possible! The same goes for tragic backgrounds, they can work but you have to go through [that magic word] RESEARCH to make sure it effects them realistically.
I absolutely hate it when the Mary Sue has the power to make characters I like retardedly OOC! Oh yeah sure, make Sephiroth into a lovey dovey wimp who'll give you 3 children and a house with a white picket fence. Nrrrrnn, brain does not comply! x___x
